4.  After the presentation, if time permits, we'll then answer your questions. Because we have limited time, please limit your answers to 3-5 minutes so we have enough time because some members have to leave at the end time.

Some members may be available after the meeting like before so you can get more clarification then.

Last Month

Dexcom Rep Linda Nunes. along with Clinical Nurse Educator Khadija Thomas. discussed the benefits of the Dexcom G6 sensors. Khadija went through the features of the Clarity App. For those without a smartphone, the Clarity App can be created on the Dexcom website. For ease of sharing data with your doctor's office, you can request a 12-digit share code. You only need to do this once for your doctor's office to be able to access your information for appointments.

For fewer night-time alarms, check your settings for alarm volume. Go to Device Settings > Options > Sound Options. Your pump has four options from vibrate to high volume. There are eight categories of Alarms, Alerts, and Reminders that you can choose to set the sound volume. There is also the ability to turn off Pump Alert and Reminders under My Pump. Check out these very helpful features for reducing pump alarm noise.

Don't forget to order a supply of the Dexcom Overpatch. The link is on the pump club blog in the Manufacturers page under Dexcom. This free helpful product from Dexcom helps keep the sensors attached for the 10 days of sensor use.

For those with skin issues who use devices with adhesive, check out the Facebook group for those with adhesive issues. Helpful tip from a pumper to help reduce skin irritation: Take the tabs off the sensor and put the code tab in safe place. Then let the sensor "air out" for at least two hours. Recommended: Use spray-on Flomax along with DuoDerm extra thin for a skin barrier under the sensor to prevent skin rashes.

The G7 will hopefully be available during the 1st quarter of 2022. This is. of course. pending FDA approval. You will be switched to the G7 when you expect your G6 transmitter is going to expire so keep that in mind when ordering sensors.

Last Month

Tandem Pump Trainer Kristy Castillo, RN, CDE, gave a comprehensive overview on the Control IQ Pump features while Moriah fielded questions in the chat room.

Kristy made several interesting points:

• Using Control IQ can increase your time-in-range by 10%.

• Control IQ will help you and your doctor reach the standard of care goal of 70-180% time-in-range.

• The Control IQ algorithm predicts your blood sugar 30 minutes ahead then adjusts your basil rate to reduce or increase your insulin for greater time-in-range.

The pump has two helpful modes:

• Exercise Mode: Make sure to turn this on 1-2 hours before starting intensive exercise.

• Sleep Mode: This great feature maintains stable blood sugars overnight. If your basil rate shuts off nightly consider adjusting your nighttime basil rate. Set sleep mode for a minimum of five hours. For a restful night's sleep consider which alarms and alerts can be set to low or vibrate (My Pump > Options > Alerts & Reminders).

Other helpful points:

• Go to Options > My CGM > CGM Alerts to manage CGM alarms. 

• Go to Options > Device Settings > Sound Volume to control sound volumes.

• Set up multiple basil rate settings for managing stress, shift work, steroids, weekend schedules, altitude, alcohol use, and hormonal changes.

• Consider treating lows with less carbs since Control IQ prevents/reduces severe lows.

• Bolus timing with meals remains important to avoid after-meal spikes. 

• Tandem offers five types of infusion sets. Check out the tandem YouTube videos to learn the features of each one. Have your doctor write your infusion set prescription for any type, not a specific type, so you can order different types if desired. You can request samples of the different types from Tandem to see which one you prefer before ordering from your supplier. 

• To keep your pump charged, connect it to your charger while showering.

Last Month

Amanda Zarling, the San Diego Abbott Libre Diabetes Sales Specialist, updated us on the Freestyle Libre 2. The Libre sensor has a 14-day lifespan with optional alarms. The  reader alerts can be set between 70 to 240 and can be set to vibrate or beep. The waterproof Libre sensor can be worn on multiple body locations with the most popular being the upper arm and abdomen. The reader uses Bluetooth and needs to stay within 20 feet to communicate with the Libre sensor. The sensor stores 8 hours of data so none would be lost if the reader is out of range of the sensor. The Libre sensors are covered under most insurance prescription benefit and are approved for children. If you are on Medicare, you must be using insulin to receive insurance coverage. We must announce, with great sadness, that Laurie Kincaid, a long-time member of the Pump Club, passed away on May 9th. In recent years, Laurie attended our meetings with Daisy, her Diabetes Alert Dog. Laurie helped schedule speakers after John Rodosevich retired from the Pump Club. Laurie worked at a travel agency before taking on, for over a decade, full-time caregiving duties for her parents.

In the past few years, Laurie faced many medical challenges and had the loving support of fellow pumper Patrick Neal and her many good friends. LAST MONTH: John Walsh had a great presentation on research regarding complications and the treatments and preventative actions people with diabetes can take. A link to John's presentation will be posted on the Pump Club blog soon.

John is working on a new book regarding complications, prevention, and treatments. His presentation was an overview of the book's content. High-glucose levels cause a number of health issues such as cardiac, arterial damage, and loss of kidney function. Oxidation caused by high BGs causes inflammation that, in turn, causes the cellular damage. Controlling blood sugars with greater time-in-range is very important (pumps are a great help to achieve this goal). The main goals for reducing complications are lowering blood sugars, lowering blood pressure to under 120, taking a statin medication, eating a healthy varied diet, and exercising. John recommends a number of supplements to help counteract cellular damage. We will let you know when the presentation link is available.

LAST MONTH: Open Discussion was attended by 15 Pumpers. Most of the Pumpers are now vaccinated! Hydrate well before having the vaccine to help avoid side effects.

Topics Discussed: Update your T Connect App to reduce the battery drain on your phone.

Tandem Pump users should upload the new Control IQ software.  To check your version, go to Options, click on My Pump, scroll down to and click on Pump Info, then scroll down to t:slim Software. Check which version you have: Moonight 7.3 or Moonlight 7.4. To get the update if your pump is in warranty, go to the Tandem website, take the tutorial, then download the update. Plan to download the software update when you need to make a cartridge change.

Alternate infusion set locations were discussed.  The best options are: Outside of upper thigh, upper arms, and upper backside.

THIS MONTH:  Open Discussion We continue to hold our Pumpers meetings via ZOOM in 2021. Bring your questions, issues and comments. For Pumpers having blood sugar or other pump issues, please take a screen shot of your pump/BG issue for sharing with the group.

LAST MONTH: Shores Salter, Omnipod Territory Manager, and Lauren Weiler CDE, Clinical Services Manager, gave a comprehensive presentation of the Omnipod pump. The Omnipod is currently available with the Euros or Dash controller. The 3rd generation Euros uses radio waves and the 4th generation Dash uses Bluetooth technology. To check out an Omnipod contact Shores to request the Promise Program, a 45 day trial. The Omnipod PDM includes the Calorie King for help with carb counting and, with the Dash, can share data with up to 12 people. The Omnipod 5 is expected in the near future and will be partnered with the Dexcom CGM. Our website has the contact information for Shores and Lauren.

After the Omnipod presentation we had a brief open discussion where a very important topic was brought up. When filling out the DMV form do you need to answer YES to the question regarding diabetes and health issues? For a detailed answer check out Kriss Halpern's website: diabetesattorney.net.

Kris is a Type 1 lawyer that has discussed this issue at TCOYD events. On his website, scroll down to the end of his home page and you will find Diabetes and DMV articles. Click on the diabetes and license-renewal article to get a legal description of the circumstances required for answering NO to the Health question you think should require a YES. All the DMV articles are very interesting and helpful.

T1D Now on C-19 Vaccine Eligibility List

San Diego county has released a document called Eligible High-Risk Medical Conditions and Disabilities. Our favorite condition, Type 1 diabetes mellitus, appears as the very last eligible condition at the bottom of page 2.

To get vaccinated, you first have to try your healthcare provider, then, if that isn't possible, go to the county.

Second, you have to prove you live or work in San Diego county and you have Type 1.

LAST MONTH: Our Open Discussion was attended by 16 Pumpers.

About 1/3 of the Pumpers have been able to be vaccinated. Hopefully more have been successful this month. I can recommend the Sharp Coronado vaccination site. It was worth the drive with no traffic or parking hassles. Topics discussed included hearing aids (Costco was recommended as a good vendor), prescription copay cards for reducing our insulin copay costs, insulin companies low-income programs, and the replacement pump for the Medtronic 670 pump.

John Walsh is our next speaker on May 10th.

4.  After the presentation if time permits, we will then answer questions brought up.  Because we have limited time, please limit your answers again to 3-5 minutes so we have enough time as some members have to leave at the stated end time.

Some members may be available after the meeting like before so you can get more clarification then.

Be respectful of person speaking, we will MUTE those that interrupt! Side conversations can be held while someone is speaking via a text chat window. There is a feedback window on the bottom bar. THIS MONTH:  Open Discussion. We continue to hold our Pumpers meetings via ZOOM in 2021. For Pumpers having blood sugar or other pump issues, please take a screen shot of your pump/BG issue for sharing with the group.  Let us know what questions you are having.

LAST MONTH: Chloe Diez from Mannkind spoke about using Afrezza Inhaled Insulin. The easiest way to receive approval for an Afrezza prescription from your insurance company is through Chloe or one of the Pharmacies that stock Afrezza. Both are available to work with insurance companies for prescription approval.  Chloe's contact information is on the Pump Club website. Recommended techniques for using Afrezza: Keep your ead upright while angling inhaler down per instructions. Drink a sip of water to prevent coughing and use at room temperature.

THIS MONTH: Chloe Diez from Mannkind updates us on the latest on Afrezza inhaled insulin. If time permits, we will have a short Open Discussion after the presentation. For Pumpers having blood sugar or other pump issues, please take a screen shot of your pump/BG issue for sharing with the group.

 

LAST MONTH: A festive December meeting included 14 Pumpers in holiday attire. Topics included managing heart and kidney disease, appreciation of diabetes technology, alternate locations for sensors, and basal rates for exercise. We also learned that new information on prescription drug interactions with CGM sensors is now included in the sensor instructions.
